I, being a parishioner of Church of Scientology London, hereby authorize repeating donations to the Church every week.
I authorize the use of my credit card/debit card for my repeating donation. I understand that the authorization for direct debits in the amount indicated above will remain in effect until it is revoked in writing to the Church.
I also understand and acknowledge that all past, present or future donations I have made and will make to the Church are the property of the Church to use at its discretion for its religious and charitable purposes. I understand that donations to the Church are generally not refundable and I am bound by the policies of the Claims Verification Board.
